Between the time of the request for readback and the next question on the record, somebody spoke, even if just to say "off the record." Just put whatever that is in there and no parenthetical indicating no readback because the absence of a "question read" parenthetical is enough of a clue that nothing got read back.

Not that I know of -- usually what happens is opposing counsel, or the witness, say something making the read-back unnecessary. If that's on the record, no parenthetical is necessary.
