statement under oath title page

I took two statements under oath. The case was about a title company gathering information from the homeowners. The seller did not disclose something to them. There's no claim or lawsuit filed, so they had no type of caption or anything like that to copy

I know generally statements under oath have just the In Re: whatever as the title page. Would you put the person's name being deposed or what? I've only done one other of these, and I can't find the file to see what I did. Thanks in advance.
