Cancellation Terms on Order Forms

The post re the lost copy got me thinking.

Does anybody have language on their copy order forms for terms of cancellation? And if not, why not?

It seems like if we want to be able to go into court and show the judge why we should be paid, that having cancellation terms on signed forms would further our cause.
