I'm a small firm and I work with other reporting firms as well. Some of these firms aren't picky about their worksheet. Is there a worksheet out there that you love to fill out? If so, can you send it to me? I'd really appreciate it.

Any suggestions on what should/should not be in a worksheet that I make myself?

I like it. Can you cut and paste info into it? Do you think it would be beneficial to create a PDF that a reporter can fill in?

yippee!!! I'll save it on my computer.
Cutting & pasting info is the greatest. To make sure it looks the way it should on the receiver's end, I create a PDF from the Word doc and that's what I send to the agency. I use this:
