Does anyone have a cert of non-appearance format where the attorney didn't put anything on the record, but wants a cert anyway???

LOL! Just joking, Angie. I was told by an owner of a court reporting company in my area that in Florida, as one example, they call it a "CNA" (Certificate of Non-Appearance).

He said that you just create a title sheet, as you would with any deposition, including the caption of the case and all the case information, et cetera.

Then, instead if the Court Reporter Certificate," you would have a CNA page, which would state something like this:

I, Jennie of CSRnation, Court Reporter, appeared at such and such date at the law offices of such and such and waited from 9:30 a.m. to 10:30 a.m., when thereafter the deponent, Tiger Woods, did not appear.

Signed ____________________________

You can probably adjust the wording to suit your individual circumstance. HTH! :-)
