I was reporting a telephonic hearing of a motion hearing and the judge ordered part of it to be sealed.

It's been years since I've done a sealed transcript and I can't find what format I used then (it was a hearing and I probably invented one, to tell the truth).  But this is to be filed in California Superior Court.

Can someone provide the heading and blurb, etc. for noting the beginning of the sealed portion and then the same for when the sealed portion ends?

I recently had one for a deposition where financial documents were ordered sealed within the transcript. 

That portion of the transcript that is sealed is placed within the original transcript with the exception of it being placed within an envelope, a title page with the style etc... and a description of what is within the sealed envelope...pages 4-12 (or whatever) at the Motion Hearing of John Doe, heard on 01/01/12, ordered sealed by the Court.

or something similar.  

Put the title page inside a window envelope, hole punch it, and treat it like additional pages.  

Put a parenthetical to introduce the Court's instructions for the sealing of the information.  I use a gold seal with my stamp on the gold sticker seal to seal the envelope.  

 

(Whereupon, the Court ordered the following portion of testimony to be sealed.)  Just an idea...
