I'd say the first one is correct.  I've never heard of "medicolegal".

Both are correct.  It depends on what is actually said, right?  "Medicolegal" is indeed a term that is increasingly in use to describe a number of things, such as legal proceedings involving, naturally, medical malpractice or that have a medical aspect.  It's used to describe the reports that medical experts are paid to give for defendants or plaintiffs.  "Medicolegal" is a word and I get it all the time.  It pertains to legal aspects of the practice of medicine.  So, depending on what is said is what you put!   

I googled it, and that word does exist and means what is implied in the depo.  The way people talk so quickly, I can see it being extremely difficult, if not impossible, to ascertain whether they're saying medical/legal or medicolegal.  I've just always written it medical/legal, but was wanting other people's opinions as to whether this could be written both ways and be correct.  Thanks!

It's hard to decipher what was said anyway so MEL/LEL is in my PD as medicolegal and that's what I always go with.
