If an exhibit was attached to an affidavit of nonappearnace how would you format that? where would the index page go or how would you mark the exhibit in an affidavit?

The certificate/affidavit of nonappearance is formatted in the same way a regular transcript would be except the title page would say "Reporter's Transcript of Proceedings" or "Scheduled Deposition of" or whatever your firm's preference. The exhibit would be marked and indexed just like it would be in a regular depo also.

I put "Certificate of Nonappearance" on the cover page.  You mark the exhibits like you would normally mark exhibits and have an index the same; nothing really changes.  You do have the transcript that looks different, however.  The reporter has her/his little read-on about they appeared at the office of blah, blah for the taking of blah blah and after Mr./Ms. so-and-so didn't show up, the following proceedings were had.......
