Hi all ~ I'm getting a little bogged down w/ my (As read) parenthetical. I have a depo of a Filipino nurse. She is constantly reading from her investigative file (she works as an evaluator for the Dept. fof Pub. Health). She has a pretty good accent so as she reads, she is either leaving off an "s" or saying a different word or dashing herself off. Or she'll say "Aug. '06" when the doc says "August 22, 2006."
Here's another example:
"Dr. XX delivered the notice of certification but did not explain [to patient] -- Patient 1" etc. The brackets are also what she said but aren't in the document ~ she just was reading it wrong.

So I guess my question is, do I put (as read) everytime she's reading from a document, even if it's just one sentence? What about when she gets it right?? She gets most wrong, but there are a couple that are correct. The (as read) is looking strange now..

oh, I forgot to add/ask ~ she also interprets as she reads (like most dr.s do, but this is bad)
i.e. chart says: "dx, arrow, MI ?, arrow, furth. eval. 5250, arrow, inpatient med. floor"
AND this is what she reads: Diagnostic workup for possible MI; placed on 5250 for further evaluation; admitted into inpatient medical floor.
At this point, I wasn't even quoting b/c it's so far off from what's in there, but then the atty asks: Where are you reading from? And she answers Pg. blah, Line blah ~ no further explanation from either that she isn't "reading" ~ ughhh!

Do I leave it unquoted??

I woulod transcribe it as I heard it, making believe I didn'tt have the document. Either use quotation marks or the [as read] parrenthetical.
Thank you both ~ I over-analyze when I have the document and can see how not perfect it was "read". I like the idea of pretending I don't have the document ~ I'd get done w/ the transcript much faster ;))
